Liver Cirrhosis in Pune: Stages, Symptoms, Diagnosis and What Treatment Can Do

Cirrhosis is not a single moment — it is the end result of years of injury, the point at which the liver's remarkable capacity for self-repair has been exhausted and replaced by scar tissue. It is the common destination of multiple diseases: untreated hepatitis B, alcohol-related liver disease, advanced NAFLD/NASH, autoimmune hepatitis, and others. It is also, critically, not a point of no return. What is possible — and what is not — depends entirely on what stage of cirrhosis you have reached, whether the underlying cause can be treated, and whether complications are being managed by a team that understands liver disease at depth.

What Happens in Cirrhosis: The Basic Biology

The liver has extraordinary regenerative capacity. Hepatocytes (liver cells) can replicate to replace damaged tissue, and the organ can maintain near-normal function even with 70–80% of its cells damaged, as long as the remaining architecture is intact. In cirrhosis, this architecture is disrupted. The normal liver lobules — the functional units — are replaced by fibrous scar tissue and abnormal regenerative nodules. The result is a liver that is structurally distorted, with impaired blood flow through the portal system and dramatically reduced functional reserve.

The consequences of this structural failure fall into two categories: hepatic insufficiency (the liver cannot perform its biochemical functions) and portal hypertension (increased blood pressure in the portal venous system, which feeds the liver, causing blood to back up and find alternative pathways — leading to varices, ascites, and splenomegaly).

Compensated vs Decompensated Cirrhosis: The Critical Distinction

Compensated cirrhosis: The liver retains enough functional reserve to perform its essential tasks. Patients may be entirely asymptomatic, and the cirrhosis is often discovered incidentally on ultrasound or when routine bloods show low platelets or a mildly elevated bilirubin. The 10-year survival in compensated cirrhosis is approximately 50% without decompensation, particularly with appropriate management of the underlying cause.

Decompensated cirrhosis: The liver can no longer compensate. Decompensation is defined by the appearance of complications: ascites (fluid in the abdomen), variceal bleeding (rupture of dilated veins in the oesophagus or stomach), hepatic encephalopathy (confusion and altered consciousness from toxin accumulation), or jaundice from hepatic insufficiency. Once decompensation occurs, the 1-year mortality risk increases substantially. The goal shifts from prevention of complications to management of existing complications and evaluation for liver transplant.

Common Causes of Cirrhosis in Pune

How Cirrhosis Is Diagnosed and Staged

Diagnosis of cirrhosis combines clinical assessment, blood tests, imaging, and where necessary, tissue sampling:

• Blood tests: Full LFT (albumin, bilirubin, PT/INR particularly important), FBC (low platelet count is a sensitive early marker of portal hypertension and hypersplenism), renal function, viral hepatitis markers
• Ultrasound abdomen with Doppler: Assesses liver texture and size (coarse, nodular liver in cirrhosis), spleen size, ascites, and portal vein flow
• FibroScan (transient elastography): Non-invasive measurement of liver stiffness — a score above 12–14 kPa in most contexts indicates advanced fibrosis or cirrhosis

What Treatment Can Do

Compensated cirrhosis: Treating the cause (antiviral therapy for HBV, complete alcohol abstinence for ALD, metabolic management for NASH), surveillance for HCC every 6 months with ultrasound, upper GI endoscopy to identify and band varices before they bleed, surveillance for hepatocellular carcinoma, dietary optimisation (adequate protein — cirrhotic patients often have protein-calorie malnutrition despite appearing to eat adequately).

Decompensated cirrhosis: Treating each complication specifically — diuretics and dietary sodium restriction for ascites, therapeutic paracentesis (drainage) for tense ascites, endoscopic variceal banding and beta-blockers for varices, rifaximin and lactulose for hepatic encephalopathy, antibiotics and prophylaxis for spontaneous bacterial peritonitis. And the ongoing question of transplant evaluation.

Can cirrhosis be reversed?

Established cirrhosis (F4 fibrosis on histology) was historically considered irreversible. However, emerging evidence — particularly in patients treated with antiviral therapy for hepatitis B — shows that sustained viral suppression can produce significant fibrosis regression, with some patients improving from cirrhosis to advanced fibrosis histologically. Whether this constitutes true reversal or remodelling is debated, but the clinical message is clear: treating the underlying cause aggressively, even in established cirrhosis, changes the trajectory significantly.

How long can you live with liver cirrhosis?

Prognosis varies enormously based on stage, cause, and management. In compensated cirrhosis, 10-year survival is approximately 50% without decompensation, particularly with appropriate management of the underlying cause.
